初診肥胖2型糖尿病患者血清betatrophin水平與胰島β細(xì)胞功能的關(guān)系

[摘要] 目的 探討初診肥胖2型糖尿病患者血清Betatrophin水平與胰島β細(xì)胞功能的關(guān)系。方法 方便選擇2014年1月—2015年1月初診肥胖2型糖尿病患者100例為研究對象(觀察組),另選擇同期該院體檢健康者(NGT)者100名為對照觀察(對照組)。測量人體指標(biāo),檢測相關(guān)代謝血生化指標(biāo),采用酶聯(lián)免疫吸附實(shí)驗(yàn)(ELISA)法測定空腹血清Betatrophin水平,采用直線相關(guān)分析研究betatrophin與胰島β細(xì)胞功能、體測及代謝指標(biāo)的相關(guān)關(guān)系。結(jié)果 觀察組血清Betatrophin水平(422.5±247.60)pg/mL明顯高于對照組(316.3±293.30)pg/mL,差異有統(tǒng)計(jì)學(xué)意義(P<0.05);相關(guān)性分析提示Betatrophin與腰臀比、HOMA-IR、FPG、2 hPG、HbA1c、FINS、TG呈正相關(guān)(r=0.215、0.216、0.249、0.447、0.338、0.452、0.470,P<0.01),與HDL-C、HOMA-β成負(fù)相關(guān)(r=-0.256、-0.363,P<0.05),差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 初診肥胖2型糖尿病患者的血清betatrophin水平較NGT組明顯升高,并且與胰島β細(xì)胞功能密切相關(guān),推測其參與了糖尿病的發(fā)生。

[Abstract] Objective This paper tries to investigate the relationship on serum betatrophin concentration and islet beta cell function in patients with newly diagnosed obese type 2 diabetes mellitus (T2DM). Methods Totally 100 patients with newly diagnosed obese type 2 diabetes in the hospital from January 2014 to January 2015 were selected as the observation group and 100 healthy people at the same period were convenient selected as the control group. Anthropometrics and blood biochemical indexes of metabolism were measured, and serum betatrophin concentration was determined with ELISA. Correlation between betatrophin and anthropometrics and biochemical indexes was linear correlation analyzed. Results Serum betatrophin level was (422.5±247.60)pg/mL vs (316.3±293.30)pg/mL significantly increased in the observation group compared with healthy controls (P<0.05); the correlation analysis suggested that serum betatrophin level in all participants was positively correlated with waist hip ratio, HOMA-IR, fasting plasma glucose, 2 h postprandial plasma glucose, glycated hemoglobin A1c, fasting serum insulin, and triglycerides(TG) (r=0.215、0.216、0.249、0.447、0.338、0.452、0.470,P<0.01), and negatively related to the high-density lipoprotein-cholesterol, HOMA-β(r=-0.256,-0.363,P<0.05), with significant difference(P<0.05). Conclusion The serum betatrophin level with newly diagnosed obese type 2 diabetes mellitus is higher, and is closely related with islet beta cell function. It may be involved in the development of diabetes.

眾所周知,2型糖尿病的特征為外周胰島素抵抗、胰島素代償性分泌增多,最終導(dǎo)致胰島細(xì)胞功能衰竭[1]。而病態(tài)肥胖是2型糖尿。═2DM)和胰島素抵抗(IR)病生理過程的主要參與者[2]。大量研究表明,促代謝因子Betatrophin參與胰島素抵抗、肥胖、T2DM等多種代謝性疾病,具體機(jī)制尚未闡明。該研究方便選取2014年1月—2015年1月收治的該院門診或住院新診斷肥胖T2DM患者100例及該院同期體檢健康者(NGT)100名為研究對象,旨在探討初診肥胖2型糖尿病患者和NGT人群血清Chemerin水平變化,探討其與肥胖、T2DM、IR的關(guān)系及其可能機(jī)制,為糖尿病的治療提供新靶點(diǎn)。
