ADAM—15在WHOⅠ級(jí)腦膜瘤中的表達(dá)及其與侵襲性的相關(guān)研究

【摘要】 目的:探究ADAM-15在WHOⅠ級(jí)腦膜瘤中的表達(dá)情況,進(jìn)一步分析其在WHOⅠ級(jí)腦膜瘤侵襲性生長(zhǎng)中的意義。方法:收集63例WHOⅠ級(jí)腦膜瘤標(biāo)本(33例有侵襲性,30例無侵襲性)為試驗(yàn)組,21例正常腦膜組織為對(duì)照組。通過免疫組織化SP法檢測(cè)去整合素-金屬蛋白酶15(A Disintegrin and Metalloproteinase 15,ADAM-15)在上述不同腦膜組織中的表達(dá)情況。結(jié)果:試驗(yàn)組ADAM-15的陽性表達(dá)率高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義( 字2=17.296,P<0.001)。侵襲性組ADAM-15的陽性表達(dá)率高于非侵襲性組,差異有統(tǒng)計(jì)學(xué)意義( 字2=8.376,P=0.004)。侵襲性組ADAM-15的陽性表達(dá)率高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義( 字2=24.661,P<0.001)。非侵襲性組ADAM-15的陽性表達(dá)率高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義( 字2=6.801,P=0.009)。結(jié)論:ADAM-15在WHOⅠ級(jí)腦膜瘤侵襲性生長(zhǎng)中起一定作用。

腦膜瘤是顱內(nèi)常見的良性腫瘤,約占原發(fā)于顱內(nèi)腫瘤的30%[1]。腦膜瘤通常為良性腫瘤,但其具有廣泛的臨床特征和組織學(xué)分類,根據(jù)WHO中樞神經(jīng)系統(tǒng)腫瘤的分類[2],將腦膜瘤分為三個(gè)級(jí)別共15個(gè)亞型,良性腦膜瘤(WHOⅠ級(jí))復(fù)發(fā)和侵襲性生長(zhǎng)的風(fēng)險(xiǎn)低,非典型腦膜瘤(WHOⅡ級(jí))更可能復(fù)發(fā),而間變性(惡性)腦膜瘤(WHOⅢ級(jí))復(fù)發(fā)的可能性最大和/或具有更強(qiáng)的侵襲性。良性腦膜瘤在組織病理學(xué)方面無明顯差別,但有一部分良性腦膜瘤細(xì)胞表現(xiàn)出向周圍組織浸潤(rùn)生長(zhǎng)的侵襲性生物行為。腦膜瘤目前的治療手段仍以手術(shù)切除為主,雖然良性腦膜瘤理論上手術(shù)全切可以治愈,但是部分良性腦膜瘤因其對(duì)周圍組織的浸潤(rùn)而無法行全切手術(shù),部分腦膜瘤即使手術(shù)全切,術(shù)后也有一定的復(fù)發(fā)率。研究良性腦膜瘤侵襲性生長(zhǎng)的相關(guān)因素,尋找有效的診斷及預(yù)測(cè)其侵襲性的生物學(xué)指標(biāo)非常重要。去整合素-金屬蛋白酶15(A Disintegrin and Metalloproteinase 15,ADAM-15)是一種Zn2+依賴的Ⅰ型多結(jié)構(gòu)域跨膜內(nèi)肽酶,屬于ADAM家族20多個(gè)成員中重要的一員[3-4],活化的ADAM-15參與細(xì)胞生長(zhǎng)以及細(xì)胞外基質(zhì)(ECM)中Ⅳ型膠原和明膠的蛋白水解[5-6]。ADAM-15參與了人類許多疾病的發(fā)展,尤其是在腫瘤的侵襲性生長(zhǎng)中起非常重要的作用。本試驗(yàn)通過檢測(cè)ADAM-15在WHOⅠ級(jí)腦膜瘤中的表達(dá)情況,研討ADAM-15在WHOⅠ級(jí)腦膜瘤侵襲性過程中的作用,為臨床提供有價(jià)值的WHOⅠ級(jí)腦膜瘤侵襲性生長(zhǎng)的預(yù)測(cè)因子,現(xiàn)將研究結(jié)果報(bào)道如下。
