聯(lián)合檢測血清鐵蛋白、葉酸、維生素B12、乳酸脫氫酶在鑒別診斷巨幼細(xì)胞性貧血中的意義

摘 要:目的 探討聯(lián)合檢測血清鐵蛋白,葉酸,維生素B12,乳酸脫氫酶在鑒別診斷巨幼細(xì)胞性貧血中的意義。方法 2015年1月~2017年2月經(jīng)萍鄉(xiāng)市人民醫(yī)院骨髓細(xì)胞學(xué)形態(tài)和臨床確診的,分組為巨幼細(xì)胞性貧血組34例,缺鐵性貧血組53例,再生障礙性貧血組25例,骨髓異常增生綜合癥組36例,另隨機(jī)選取60例體檢健康者為正常對(duì)照組,回顧性分析不同組別患者的血清鐵蛋白,葉酸,維生素B12,乳酸脫氫酶水平變化。結(jié)果 巨幼細(xì)胞性貧血患者鐵蛋白輕度升高,與正常對(duì)照組及MDS組差異不明顯(P>0.05),與IDA組及AA組相比較,差異具有統(tǒng)計(jì)學(xué)意義(P<0.05);MA組葉酸,維生素B12水平降低幅度明顯,與正常對(duì)照組及其他類型貧血組水平相比,差異明顯(P<0.05);MA組乳酸脫氫酶水平明顯增高,與正常對(duì)照組及其他類型貧血組變化水平相比較具有明顯差異(P<0.05)。結(jié)論 聯(lián)合檢測血清鐵蛋白,葉酸,維生素B12,乳酸脫氫酶可為鑒別診斷MA提供有效的依據(jù),對(duì)減少誤診率,減輕患者身心及經(jīng)濟(jì)負(fù)擔(dān)方面具有積極的意義。

Clinical Significance of Combined Determination of Serum Ferritin,F(xiàn)olic acid, Vitamin B12 and Lactate Dehydrogenase in Differential Diagnosis of Megaloblastic Anemia

Abstract:Objective To explore the significance of combined detection of serum ferritin, folic acid,vitamin B12,and lactate dehydrogenase in the differential diagnosis of megaloblastic anemia.Methods From January 2015 to February 2017,the patients were diagnosed with bone marrow cytology and clinical diagnosis at the people"s hospital of Pingxiang.They were grouped into 34 cases of megaloblastic anemia,53 cases of iron deficiency anemia,and 25 cases of aplastic anemia.There were 36 cases of dysplasia syndrome group.Another 60 healthy subjects were randomly selected as the normal control group.Serum ferritin,folic acid,vitamin B12 and lactate dehydrogenase levels were analyzed retrospectively in different groups.Results Ferritin in patients with megaloblastic anemia increased slightly,and the normal control group and MDS group was not significantly different(P>0.05),compared with the IDA group and AA group,the difference was statistically significant(P<0.05);Group MA,folic acid,vitamin B12 levels decreased significantly, compared with normal control group and other types of anemia group were significantly different(P<0.05);group MA lactate dehydrogenase levels were significantly increased,and the normal control group and other types of anemia group level changes compared with significant difference(P<0.05).Conclusion Combined detection of serum ferritin,folic acid,vitamin B12 and lactate dehydrogenase can provide effective evidence for differential diagnosis of MA,and it has a positive significance in reducing misdiagnosis rate and relieving patients" physical and mental economic burden.
